Road Captains Only
The following 4 forms may be needed for road captains and are available for download as PDFs:
1) Release form for adults (required for all participants 18 or over for OPEN events and for any particpant 18 and over who has not signed a Chapter Enrollment and Release Form...for CLOSED events)
2) Release form for minor (required for all minors, under the age of 18, for all events, each time, there is no annual release form for minors)
3) Minor's Assumption of Risk (required for all minors aged 12 or older but less than 18, IN ADDITION TO the release form for minor referenced in #2)
